Mental Health Court - Targeted Case Manager

Provide client centered targeted case management to Mental Health Court clients who meet the criteria for services at Okeechobee County.

• Provide TCM services to clients from the Mental Health Court (MHC) grant.
• Utilizes the SEFBHN coordination of care module to promote communication across providers of MHC clients.
• Utilizes wraparound case management methodologies and strategies to provide intensive, individualized case.
• Participate in weekly care coordination staff meeting with MHC Care Coordination Team.
• Collaborate with MHC of Okee to facilitate SOAR application process.
• Possess procedural knowledge of community resources.
• Assess clients needs.
• Provide linkage to appropriate community resources; etc.

Full time / Salaried Position.

Skills/ Requirements

• Minimum of a Bachelors Degree (required)
• CBHCM Certification required. Must be able to obtain in 6 months.
• Experience working with targeted population (preferred).
